Join us to learn about New York State’s Social Care Network Program, opportunities to address the health-related social needs (HRSNs) - such as food insecurity, housing instability, and lack of transportation - of Medicaid clients, and agency reimbursement for participation in the program.

Background: The NYHER (New York Health Equity Reform) program is a New York State Medicaid initiative aimed at improving health equity and reducing health disparities for Medicaid members.
